preferred date, imaging area, patient's name, and date of birth.

Please download the Patient Referral Document, fill in the medical information,

and send it to our hospital via fax.

　Patient Referral Document

Please instruct the patient to bring the original copy of the

Patient Referral Document and their health insurance card on the day of the examination.

Please have the patient register by their reservation time.

After registration, a preliminary interview will be conducted, and the examination will begin.

The captured images will be recorded on a CD and handed over after payment.

At a later date, the interpretation results will be mailed to the referring medical institution.
