This is a system that allows for smooth coordination of medical consultations and hospital admissions by registering necessary information in advance, for when patients receiving in-home care become unwell and it becomes difficult to manage at home.
At Nagitsuji Hospital, you can consult with us about everything from an explanation of the system to how to proceed with the registration procedure.

Support at Nagitsuji Hospital

For those who register with our hospital,
we provide a dedicated 24-hour contact number.
We will stay close to you and support you through any anxiety during nights, weekends, or times when you cannot reach your primary care doctor.

Recommended for:

  • Recently feeling unwell and anxious about where to go for consultation or admission in an emergency.
  • Currently receiving in-home care and want to organize the emergency response flow, including for family members.
  • Want to ensure seamless coordination between medical and nursing care, including life after discharge.
  • Unsure about "primary care doctors" or the system, and just want to consult first.

Residents of Kyoto Prefecture aged 65 or older
who are receiving in-home care.

What You Can Consult About at Nagitsuji Hospital

Please consult us for everything from system explanations to procedures

1. Consultation for a "Primary Care Doctor"

Please don't worry if you "don't know where to consult" or "haven't decided on a primary care doctor." We will listen to your situation and help you organize your future preparations together.

2. Guidance on Application Procedures

We will clearly explain the preparation of application forms and the flow of filling them out.
(A portion of the application form requires the primary care doctor's entry, signature, or seal.)

3. In Case of Emergency

We will help you organize "who to contact first" when you feel unwell, so you won't be confused after registration.
* We will also provide details about the dedicated 24-hour hotline for registered patients.

How to Register

Procedures will be carried out according to your personal situation in the following flow.

Those who have a Primary Care Doctor
  • Consult your primary care doctor
  • Select a registration hospital from the "List of In-Home Care Reassurance Hospitals" in the application forms.
    (You can select up to 3 hospitals)
  • Fill in the necessary items on the "Registration Application Form"
  • Mail to the submission destination
  • Receive the registration completion notice
    (Please keep it in a safe place)

Those who are unsure about their Primary Care Doctor
  • First, consult with our hospital
    We will check your living situation, hospital visit status, and any concerns.
  • If necessary, we will coordinate to identify a primary care doctor or establish cooperation with support services (care managers, regional comprehensive support centers, etc.).
  • Once the conditions are met, we will proceed with the In-Home Care Reassurance Hospital registration.

How to Use (Flow After Registration)

When You Feel Unwell

1. First, please have a consultation with your primary care doctor.

The primary care doctor will provide initial care.

Coordination of Consultation / Admission

If the primary care doctor determines that "consultation or admission is necessary," the doctor will contact the registered hospital (such as Nagitsuji Hospital) to coordinate.

For those registered with Nagitsuji Hospital
If you cannot contact your primary care doctor during nights or holidays, please call the "24-hour hotline" provided to you in advance.
To the Registered Hospital

You will be examined at the registered hospital, and if admission is deemed necessary, you will be admitted there.

Discharge / Return Home

From the time of admission, care managers, regional comprehensive support centers, visiting nurses, etc., will collaborate to support an early return home.
After discharge, the primary care doctor and the registered hospital will continue to collaborate to provide support.

Important Notes on Usage

This system can be used when you temporarily become unwell based on a doctor's judgment.

  • Highly urgent illnesses or injuries (requiring an ambulance) are not covered.
  • Admissions for the purpose of long-term care are not covered.
  • This is not a system that guarantees immediate admission. (It will be decided based on your medical condition and bed availability.)

It is not intended for long-term care, but for an early return home. The doctor will determine the necessary hospitalization period based on the medical condition.

The support of a primary care doctor is necessary for judging the need for admission, coordinating for discharge, and continuing medical care after discharge. First, decide on a primary care doctor and then register.

If you cannot contact your primary care doctor, please contact the registered hospital directly.
Those registered with Nagitsuji Hospital can use the dedicated 24-hour hotline.
