Admission Guide (Preparation & Procedures)

About Admission Procedures

Those being admitted should fill out and sign the following documents and submit them to the 1st Floor General Reception at the time of admission.

  • Written Oath for Admission
  • Application for Provision of Medical Information (Please select the person to receive explanations in advance)
  • Consent Form for Out-of-Pocket Expenses
  • Health Insurance Card / Various Medical Certificates (If submission is delayed, you may be treated as a private paying patient)

* If you wish to use a private room or a 2-bed room with an extra charge, please let us know. (Refer to the separate sheet "Notice Regarding Out-of-Pocket Expenses" or the Inpatient Environment & Facilities page)

About Belongings

[Items to Bring]

- Medications currently being taken, medication notebook, instructions

- Any other items instructed by doctors or nurses


[Precautions Regarding Belongings]

- Please be sure to write your name on all your belongings.

- There is no shop in our hospital. Please prepare necessary items in advance.

- Please do not bring electrical appliances such as televisions and electric kettles.

- Please do not bring valuables, large amounts of cash, or fragile items.

Inpatient Set Rental Service
For hygiene management, our hospital requests that you subscribe to the Inpatient Set (clothing, nursing care sleepwear, towels, daily consumables). Please check the separate sheet for details.
About the Admission Deposit

For those using rooms with an extra charge, we will ask for a deposit of 50,000 yen.

* The deposit will be returned after settlement at the time of discharge.

About Hospital Life

Daily Flow & Care

Vital Check

10:00 (Regular), and as appropriate

Meals

Breakfast around 8:00 / Lunch around 12:00 / Dinner around 18:00

Bathing

The attending physician's permission is required. For those who need assistance, we provide special baths 1-2 times a week (suspended on public holidays).

Linen & Sleepwear Change

Once a week (changed as needed when soiled)

Diaper Change

Cloth diapers are used. In principle, bringing in paper diapers is not allowed.

Other Services (Paid)

Visiting haircut (once a month), Oral care (1-2 times a week)

Meals & Bringing in Food

We will prepare a therapeutic diet suited to your medical condition. Please let us know if you have any food allergies.

Restrictions on Bringing in Food and Drinks

  • Please refrain from bringing home-cooked food, purchased deli items, seasonings, raw food, and fresh sweets.
  • Please refrain from bringing items that are not individually wrapped or lack an expiration date.
  • Please refrain from bringing drinks other than water and tea.
  • Please refrain from "sharing" with other patients.

* The attending physician's permission is required to consume anything other than the therapeutic diet.

* If you wish to cancel a meal due to going out or staying overnight, please inform a nurse by 17:00 two days in advance.

Hospital Facilities & Communication Devices

Television & Refrigerator

Each bed has a card-operated television and refrigerator. Please purchase a card from the vending machine in the Day Room.

* Please use earphones when watching television.

Telephone

Public telephone: 5th Floor Ward Day Room

Mobile phones: Can only be used in private rooms, the Day Room, and the 1st floor (please refrain from making calls in hospital rooms and corridors).

Requests During Hospitalization
  • The premises are "completely non-smoking", and "no alcohol" is allowed during hospitalization.
  • Lights out is at 21:00.
  • Please refrain from entering other hospital rooms or other wards.
  • When going out or staying overnight, be sure to get permission from your attending physician (please carry your permit).
  • In the event of an emergency or disaster, please follow the instructions of the hospital staff.

* If we cannot obtain your cooperation regarding recuperation during your hospitalization, you may be asked to be discharged.

Surgery, Visiting, Discharge, and Others

For Those Undergoing Surgery

An oral check will be performed at the dental clinic before surgery (to prevent tooth loss and aspiration pneumonia).

Please also cooperate with the following points.

  • Please remove nail polish and manicures before coming.
  • An abdominal binder or T-bandage may be required.
About Visiting
Notice of Visiting Restrictions
Currently, visits are limited to 60 minutes from entry to exit per visit, and visits on Saturdays and Sundays require a reservation. (Reservations are not required on weekdays.) For detailed conditions, please check the "About Visiting" page or contact the hospital.
Payment of Hospitalization Fees & Discharge

[Payment During Hospitalization]

Bills are closed at the end of each month, and an invoice is issued around the 15th of the following month. Please pay by bank transfer by the end of that month.


[Payment at Discharge]

Discharge requires the permission of the attending physician.

Once you receive the invoice on the day of discharge, please pay by bank transfer within one week.


* Please check the attached document for the transfer destination.

* Receipts cannot be reissued, so please keep them safe.

Others & Parking

[Various Certificates & Consultations]

Please ask at the 1st Floor General Reception for the issuance of medical certificates, etc.

If you have any problems during your hospitalization, please consult the Staff Station or the 1st Floor General Reception.

* Our hospital strictly declines any gifts or gratuities to doctors and staff.


[Use of Parking Lot]

An exclusive parking lot is located approximately 80m west of our hospital (along Shin-Jujo Dori).

Inpatients are requested not to park their own vehicles.

- Attendants: Free only on the day of admission and discharge

- Visitors: Free up to 1 hour / Over 1 hour: 200 yen/hour
